Is this sentence grammatically correct?

sure it is grammatically correct. But this might not be correct if it were in a context(that is to so in an action environment).
"it's back" is the contracted form of It is back.
grammatically, you need a subject, a verb and an object(compement) to have a sentence aside the fact that there are simple, complex and compound sentences.
in that sentence,
it= subject
is= auxilliary verb(there are types of verbs but most teachers simply misteach that a verb is a doing word and so since "is" isnot a doing word, you will be tempted to say it is not a verb but it is. an auxilliary or helping verb for that matter.)
back= compement(object).a compement because it is not different from the subject as it talks about the subject.
this is a simple sentence.

Yes. "It's" is the contraction for "it is"

However, do not capitalize Back, unless you are using this sentence as the title of a document.
