You need to use a special type of primer, called stain sealers. One popular brand is called "Kilz". If you don't use that, the stain will bleed into your finish paint in minutes. If the stain accumulation is heavy you may possibly need two coats of sealer. Sealers can be tinted to approximate colors, but it's usually not recommended.

There is a primer that you can paint over it. Ask in the paint store. I worked at a bar and they painted the walls and needed that primer. The paint store will tell you.

You dont have to do any just a few thick coats of paint but a bucket of hot soapy water and a rag would do fine just be sure to let it dry before you paint.

if you scrub them really well first the primer b4 the paint should do it--i used to be a painter and that always worked for me.make sure you use a good cleaner though

I recommend scrubbing the walls .TSP(TriSodiumPhosphat) works good.Then prime the surface.I like to use KilZ.Finally ,the paint.You get what you pay for.
