I would say it depends on the person and the exercise. Once you don't feel anything from it I would move on to something else or increase that work out, i.e. If your doing crunches you might want to either do more crunches in less time or start doing sit ups, when you don't feel anything.

not that often. all you really need to do is to change your rep range periodically and the order of exercises performed.

the majority of the time when resistance trainers reach a plateau it's simply due to overtraining of the CNS.

do some research on linear periodization, you will never hit another plateau again..

I would say to change your program once every 3 months. That way you won't get bored and you will still be able to memorize the workouts.

depends on if you are talking about weight training or cardio.
variety keeps things interesting in cardio and will help you stick with it, I'm not a weight training person so I cant help you there
