This is a very sensitive issue that is difficult to address. I guess you could leave an anonymous note, unless it is going to cause problems at work. Breath mints will not eliminate bad breath and sucking on mints all day cause cavities. The only thing they would do is get your point across. Breath mints can be compared to this analogy:

If a dog pooped in your living room, how do you get rid of the smell?
A. Spray air freshner or Febreeze around the room or on the poo.

B. Pick up the poo (using disposable gloves of course), throw it in the garbage outside, clean up your floor.

My point is, she needs to eliminate the source of the stench to eliminate the problem.

If her mouth looked like this.... http://www.apreso.mcg.edu/Hub/NEWHUB/PeerReview/images/pooroh_small.jpg
her breath would still stink to high heaven no matter how many breath mints she sucked

I won't bother going into details about how to eliminate the smell because you know the answer and that is not the question you asked.

Her breath is worse in the morning because when you are sleeping saliva production slows down and all the bacteria that is in her mouth (from a lack of brushing and flossing) have a party in that dry enviroment and flourish, creating quite a stink. Saliva is a natural cleanser and also acts as a neutralizer for the acid that is produced by the bacteria. That's why people who tend to have a dry mouth are more prone to bad breath and cavities. Everyone has dry mouth in the morning, which is why we have morning breath, especially if we didn't brush and floss throughly before bedtime.

If you decide to leave an anonymous note perhaps you can leave a traveller size tube of toothpaste and a toothbrush instead of mints.

Send out a memo that patients have complained of a nurse with bad breath. Include in the memo things you can do (other than the obvious, BRUSH AND FLOSS YOUR TEETH) to eliminate bad breath such as

1. Keep your tongue clean - Gently brush it with a soft nylon toothbrush after you brush your teeth.

2. The Drier Your Mouth, The Worse Your Breath Gets: Drink MORE Water!

3. You probably didn't know that many Protein Supplements are really "bad breath in a bottle".

4. Keep Calm - Stress Makes your breath worse

5. Avoid breath mints and/or gum with sugar, all Bacteria love sugar. It's like Mother's Milk to them.

8. You can't kill the odor of bad breath with another odor. Why? Just think of this…if your breath smells like poop and you add peppermint to that, what do you have? Correct, Peppermint Poop!

9. Your Breath Gets Worse When you Have a Cold, Allergies, or Post Nasal Drip, so Blow Your Nose!

I have also heard that people on low carb diets who eat a lot of meat will have breath like that or people who are starving themselves to lose weight will have bad breath too (probably dehydration or ketosis).

This lady's problem can stem from several reasons. Number one is dental decay or periodontal disease. Number two can be stomach problems. Its probably the first one, if it is an ongoing problem. Get on the topic of dentists and see if she has one. Tell her that u have or have heard of a great one. Give her a name of one. Mints will only cover up the problem for a few minutes, and will do nothing for the cause.
Good luck

Advising coworkers of grooming issues is tough -
Only rare people can do it so I would suggest a polite short anonymous note saying something like . . .
"we like you , and need to let you know you have dental health issues , maybe you can use these (include some Altoids) until you can schedule a dentist visit"

letting people know is really hard, but you could give her a snack basket on her birthday with mints among other things, if she likes the mints maybe she will continue to eat them and thus solve the problem

leave a mint or something in her purse/locker, she won't know who that was but will, probably, get the message. If you want to be drastic leave mouthwash.

don't just have tic tac mints and start to use them and then offer her some of the mints after she has seen you using them.
