The functional equivalent of a Prime Minister in a parlimentary system is the Speaker of the House in the American system.
However, most Americans know what a Prime Minister is, so I'd just leave it at "Prime Minister". To use "Speaker" would introduce undue complications to your story.

What's the need? Intelligent Americans know what a prime minister and a minister are, even if there isn't one in American government, so doing so would probably end up misleading people into believing the passage (from whatever Arabic country it's from) uses the American system of government, which is incorrect.

In england the prime minister is a lot like the president or speaker of the house in the united states. With no absolute power, the prime minister must answer to the queen just as the president must answer to congress.

I supp0ose it depends on the context that Prime Ministr is being used in. Is it the head of a country, organization, etc? Then President. Is it the type of Prime Minister that is under someone such as King? Probably Vice President? Under a being like God? Then maybe a priest/pastor/bishop, etc...

Prime minister would still work. People understand what one is, even though we don't have one. Minister -> Secretary is good, but Prime Minister is understood.
You should keep it Prime Minister anyway because it's a title.

the word prime minister is widely used in the US to designate heads of foreign States. You might also use "Premier".
