If you need to keep the orig. grips on the firearm for personal reasons, then try 38 wadcutter rounds. They are only lead and don't have the recoil of standard practice rounds. They are also good for personel deffense. The wadcutter transfers their energy and expand making a good stopping round.
You could also change the grip to a larger one. Hogue or Pachmayer. I like the pachmayers better. You can get them with out the finger grooves. But since they are all rubber they hang up on clothing or purse holsters and make it hard to carry.

Since you say you have other guns I assume you are familar with shooting. But I wonder why your thumbs hurt. The "Lady Smith" has a small grip that my wife or I hold in one hand. Index at the trigger, fingers around the grip, thumb down over tips of fingers.
I have large hands and have shot the "LadySmith" and didn't notice problem. My wife has smaller hands and it doesn't bother her either.
As others have suggested ,change grips, use ighter loads, but you might take it to a gunsmith to see if there is a defect.

My wife has a Lady Smith 3913LS 9mm auto and has shot it for years. Her hand and trigger finger would hurt after shooting a 100 rounds or so of target practice.
I bought her a Glock 19 9mm for her birthday after shooting my Glock 17 which was to large for her.
She shoots the Glock now and never has hand fatigue like she did before.
The trigger on the S&W's are much heavier than the Glocks are so you could probably get a gunsmith to tune the trigger assembly to make it smoother.

You say it hurts your `thumbs' , as in 2 ?You are no doubt shooting this small handgun with Both hands.Are you shooting +p
ammo ? If so, try using regular ammo.
If not, try shooting it using only 1 hand.You don't need a death-grip
either.
I have a .454 casull that some of my friends won't even shoot.
because of the reputation it has of being `Brutal'
I shoot it all the time with 1 hand, and I think it Kicks less! Call me Crazy

You can buy a trigger grip, and new pistol grips should fix you up fine. If you still have problems with the trigger, you might be able to get some gunsmithing done to round off the trigger more.

If it is the lady smith it probaly has red wood grips, if so just get some Hogue grips for your model, there only like $20. It problem still happens use lighter loads.
