Yes, only for those who have made temple covenants (or promises) to keep the commandments.
It's not so unusual for people to wear thing to remind them of their faith -Much like some Jewish people wear hats, or some clerics wear robes or collars, we wear something too to remind us of the promises we made. Why under the clothes? mostly because we don't have paid clergy and we all need to go about our different daily employments without anything getting in the way.

You are talking about "temple Mormons." This does not apply to all Mormons. Some Mormons undergo a special initiation ceremony at the Temple. During the ceremony they are presented with the special underwear that you are asking about.

I am a "temple-going" Mormon...yes, when we are endowed in the temple, we receive garments which like have been said already are basically t-shirt, boxer type underwear for men and more feminie "t-shirt, biker shorts" for women..that probably sounds funny but they are very feminie for the women and comfortable, I love them. They are a symbol of covenants we have made...they help us remember those covenants, keep us dressing modestly and are promised to be a spiritual and physical protection for us.
I feel honored and very blessed to be able to wear the sacred garments... they are very special to those who worthily wear them. To others who do not fully understand their meaning I am sure they seem really weird...but when you understand them and respect them for what they are, they are truly sacred.
And like someone mentioned, only temple-endowed members wear them or can even buy them from church distribution centers.

Yes. Latter-day Saints who have been endowed (i.e. have been through Temple ceremonies) are bound by covenant to wear undergarments specified by those covenants. They are no different than an undershirt and boxers but with special markings (a square on one breast, a compass on the other), in addition the knee of the undershorts have the same markings. These supposedly provide divine protection.
In addition, they are the private mark of a Latter-day Saint in good standing as those who are under church discipline are generally forbidden to wear them.
