If you're a member of the deceased's immediate family, you light a yarhzeit candle and say the kaddish prayer. The candles are those small ones in glass cups with the Hebrew writing on them that you periodically see in the grocery store. You are supposed to light a candle on the anniversary of the loved one's death, on Yom Kippor, on Shavous, and (I think) on the last night of Sukkot. If you have more questions, you can write to:

The common custom is to light a candle to burn thru the day (beginning at sunset the night before). Other things one can do is to give charity in their memory and to say special memorial prayers in synagogue. Also, if it is possible, to visit the grave and recite certain psalms in their memory.
