and do it with politeness , thats what old ladies appreciate.

It depends on who the old lady is and how old she is. If she is any relation to you you have to go, and if she is wicked old you have to go. If you really don't want to paint just tell her you don't want to paint but would love to spend some time with her and keep her company, which is probably what she wants.
Sometimes the elderly are just lonely and all they want is some company and it won't kill you to spend some time with her.

Politely decline. If you aren't full into it you could hurt her feelings. The elderly really just want company and someone to talk to.
