The Jihadi Terrorists will do it just as soon as they get the chance. Israel is the most likely target; the right size bomb could wipe out as much as 80% of their country. These monsters have no moral compulsions about using such things. In their sick, twisted philosophy, anything that advances their cause and harms their enemies is allowed and encouraged. That is why it is so vital to take strong action against them now, before they get WMDs and use them. Or, imagine a small plane flying over an American city spraying plutonium dust over the whole area. Calculate the cost of that in terms of lives and money.

Nukes tend to stop themselves as previous weapons killed your enemy while nukes kill your enemy and you to. This realization is why nukes are strictly defensive in nature. To use a nuke on the offense is will only end with you giving your own people a nasty dose of radiation poisoning at the bare minimum. Thus the worry about others having nukes is mainly hype and worry that we can't push countries around like we used to. That is the true power of a nuke is to prevent another country from attacking you.
