If the cartilage is not damaged, then a combination of stretching the muscles, exercise and massaging the knee cap before exercising can keep things under control. It would be a good idea to have a physical therapist instruct you on the specifics.

If there is already cartilage damage, then the next best procedure is to drill tiny holes in the cartilage with arthroscopic surgery.

There are other more involved surgical approaches, but its a good idea to try the simple things first..
