I feel your angst... I used to be plagued the same way... one of the ideas that helped me was to run through in my mind what I see in the future if I did things one way and then if I did things another way... what would it look like a week from now if I did things this way, then that way... what it would look like in a month, a year, 5 years... this way then that way... (if only I'd known about this before I married :-})... another way that I could gauge my gut was to sit quietly for a bit and ask myself for example "do I really want this job?"... "do I really want to go out tonight?" and "listen" for my gut to go zing or thud... these ideas may sound simplistic but they work if you work them...
ultimately, I learned how to set a goal based on my desires and needs and stay on track until I achieved them... it is rare now that I need to go through the above exercises because I've trained myself to know what I want/need and stay focused until the goal's been achieved
once you have practice setting goals, the question to ask yourself when faced with such decisions is "will this action move me towards my goal/away from my goal/keep me treading water?"
hold yourself accountable to your self... often the biggest obstacle to our peace and mental wellness is ourselves
best of luck with your efforts... whatever you do try, give it a chance to work... it takes somewhere between 21-30 days to develop a new habit
remember... even when you are on the right track, if you get paralyzed, that old train can mow ya down

I don't have any advice but I suffer from some of the same anxieties so I will share my experience. Here is what I do and maybe it will help and if not please disregard it. Every day i wake up and set an intention to do God's will during the day. Each moment of the day I return to this thought when I can. Of course I get distracted. As I have a more sincere intention God's will becomes more clear to me through faith and intuition. Of course my own will, or the desires of the ego pop up. That's life. Over time my life has become more spiritualized and though I still suffer, the load has become lighter. A good indication of God's will is whether or not there is love in any given action.

I've suffered this problem my whole life. I still do. That is the reason why I right now lack the confidence to make a huge decision that would change my life around for the better. That and my fear of my mother. Confidence is the key. Although I lack confidence, because it has been repeatedly taken away from me, the few times I have experienced it, it is truly the one thing which helps you make decisions and stick with them through anything.
I truly hope this helps you, because I myself do not know how to gain any confidence....I don't know you, but your circumstances in life might be different than mine, because I have a certain person in my life who repeating steals my confidence.

Whenever you're faced with a decision making opportunity and are afraid of making the incorrect choice, just ask yourself this question: "What's the worst that could happen?" Almost always the worst is not a very major consideration, so it's hardly worth worrying about it.

Meds or brain surgery. Personally, I'd go with the meds--I use Effexor. It works most of the time.
