OK--I think you skipped a period in there somewhere, but I'm understanding that you want to know what the parents of kids already enrolled think of your center.........

If so, ask:

Do you feel your concerns are addressed timely by our staff and to your satisfaction?

What do you like best about our Center?

What do like least?

Are our hours adequate for you?

Would an evening program help your scheulde and if so, would you be willing to pay extra for that? If so, how much?

How would your rate your child's carevgiver on the following:
(Give a scale here of 1 to 5 or something)
Dress
Reaction to your concerns
Notifying you when you need siapers or wet wipes
Informing you of how's your child's day went

If your child eats lunch here at the Center, do they enjoy their meals? If not, what can we do to make your child happier?

Do you feel you 'get your money's worth' for the care provided to your child?

Have you called the Center during the day to check on your child?
If so, did we answer phone promptly? Answer your questions?

If you have special requirements for your child, such as diet or medication, do you feel you child's caregiver attends to those needs as you'd like them met? If not, how can we improve?

Do you feel your child's caregiver teaches your child to do things that are age appropriate such as blow a kiss or his/her letters?

Please tell us anything else you feel we need to know in order to nesure that your comes to a safe, loving place everyday.

Those are not in any particular order, but I think you need to let the parents know you aren't just looking for praise, but that you are actully looking for their concerns too and have the willingness to address those concerns. Also--make sure you proofread and have someone else proofread--you don't want any mistakes (I'm sure I have some misspelling up there) in your questionaire as it will reflect badly on the Center.

Also--add space for them to communicate anything else they feel you need to know that may not have been asked.

And finally--I say good for you for wanting to get such feedback from your clients!!! HIP HIP HOORAY!
