It does not specify what the income requirements are but it does state that all health care programs have "income tests" for qualification.

You have to go to Michigan FIA depending on which county you live in or will plan to live in and apply, they will run a budget depending on your household expenses and your income. I do believe the current poverty level is 19K and under per year. Dial 586)254-8087 and press O when the attendant comes on ask her where you can look up the information.
