It is also best to get all the visas you need before you leave your home country, I tried to get a visa to Russia without having official residence where I live and I was denied (I'm an American living in Europe).
If you are an American, you generally don't need a visa to go anywhere except Russia and the countries that where part of the USSR: Khazhakstan ect. You can go throughout the Middle East, but and Israeli stamp might cause you problems getting into say-Syria, and vice-versa.
You DO NOT need a visa to go to any EU country, nor Croatia and my guess is the rest of the Balkans and Turkey as well. Anyone you tells you otherwise is simply wrong!

It depends on the country. You will definately need visas to enter a foreign country but some may be obtained as entry stamps on arrival at the immigration point, others you will need to buy from the country's embassy or consulate in your own country.
Many now have visa applications on line or at least allow you to download the forms from a website.
In the European Union not all member states belong to the Shenghan(? spelling) agreement. While you may have a multiple entry visa for most of the EU states you will still need to show your passport at the border/airport and perhaps have it stamped with an entry stamp.
The United Kingdom and Ireland are not part of the open border agreement and seperate visas are required fot these two countries.
