4 years together and she won't tell a male friend of hers to quit calling in the middle of the night. I told her its him or me and she said wont choose. I should go right?

You don't mention what the relationship with this other male friend of hers is. I assume she's having sex with him or it wouldn't bother you so much.
The only answer I can offer is that you need to be honest with yourself about how you feel over this. You've been with her for four years and put up with this behaviour, but she's now saying that she won't 'choose'. Personally I'd consider that to be a choice - the choice not to consider how I feel about the situation, the choice not to make a committment to me. If I felt as though I was being disrespected by her, and was disrespecting myself if I didn't do anything about it, I'd walk. I suspect this is where you find yourself.
I realise that you don't want to go, but there's little value in being attached to a situation that won't change - and it won't because she has no reason to. She can have you both and knows you won't make good your threat to leave so she'll just keep playing. Personally I wouldn't put up with that and just accept the fact that it's time to move on.

If this has been going on for 4 years, no. If it has just started, yes. Anytime you give someone an ultimatum, you have to be prepared to accept the decision.

It all depends, What are the facts, Did she ever have any kind of emotional or physical involvement with him besides of a friendship. The decision is all yours.
