The W-2 has to be mailed to you and postmarked by January 31. However, if the company is fairly large (i.e. they have a payroll department and are using electronic software, or they have a payroll company doing their payroll), you can generally get your W-2 earlier. For example, with my company, we can get copies of our W-2 from our company's intranet site on January 15th.

Depends on the company. By law, they have to be sent out by January 31.

I usually get them aroudn the end of January. At the very latest, sometime in Februrary. If you are unsure, contact your employer and make sure they send one to you.
