First and foremost- RESEARCH your ARTIST!!!
THEN- pick a spot that is not one of your more tender areas-ie. inside of upper arm is BAD one!!
I owed a TATT shop on the beach in FLA for several years and I know that it matters more who is wielding the gun than what you might do to alleviate pain in advance. It may in fact make it so you will not be able to get the tattoo. If it is a reputable shop they will NOT tattoo you when you are obviously under the influence of drugs or alcohol.
The pain is short lived and should be fairly minimal. I do wish to add do not take anything such as aspirin prior, because it can thin your blood and make it harder (you bleed more) to give you your TATT and you do want to make it as simple and easy for your artist as possible as this will be on you permanently!

Watch them tattoo others and see the reactions of those in the chair.

They don't really hurt that much. From what I have heard from my female friends that have them on the lower back it is not that painful. I have also been told that women are less sensitive to pain there (not sure if that is true though).
Whatever you do, don't drink, that will thin the blood out and make you bleed more. If you are really looking for something, there is a thing called Emla (not sure if that is the correct spelling) cream that temporarily numbs the area that you put it on. It takes a certain amount of time to actually kick in.
But in all honesty, just grin and bare it, that is actually a good part of the process. From my experience, and I have had quite a few of them done, there is a certain feeling that I got from sitting there through the pain, that made me recognize that if I can sit through that, nothing else can really be that bad. I felt a sense of calmness, so much so that when I get them now, I can actually sit and read a book while getting them done.

DO NOT DRINK!!!!! Alcohol thins the blood and you will bleed more than usual. Most GOOD tattoo artists will no tattoo someone who is drunk or inebriated. The lower back is not so bad. I found that the healing hurt and itched more than getting the tattoo. If you must, a percoset will work wonders. It's a medication given usually for tooth pain or pain in general but make sure you take it well in advance. They can sometimes hit pretty hard if you're not used to them.
Best advice just breathe relax and make sure you are in a comfortable position from the very start.

I am getting one on my lower back in 3 weeks as well but not on the spine, on the side...first ink as well...done lots of research on this and honestly, it sounds like the best thing to do would be to take nothing. I've had a baby before so i plan to just focus on something and try to breathe deeply. I figure that the tattoo is important enough to me...to just get it and deal with the pain. For me, I am not taking anything or getting any numbing cream because you don't want the numbing cream to wear off in the middle of your tattoo and nothing is a guarantee in terms of taking away the pain. Everyone seems different. What seems like horrible pain to me might be nothing to you. You also don't want to take alcohol or pain medications because you don't know the effect on your blood (thinning). Since this design is going to be on you for the rest of your life and it's that important to you, you don't want to do anything to chance messing it up. do not take Tylenol! just as do not drink alcohol, they both thin the blood and your blood will not clot very well after your tattoo, then the ink may bleed and you will end up with one ugly tattoo. the only thing that i may suggest to minimize pain is the placement of the tattoo, size and filling make a difference too. Talk to your artist about this, they know the best

My wife has got 4 tattoo's now and the first she went cold turkey and she said it hurt. You can buy a local anasetic cream called "EMLA" which goes on the skin approx 1 hour before - take off and tattoo away. My wife has used this for her last 3 and has sweared by it. Area is numb as. And now she is planning her 5th, yes with the Emla to help.
My wife is a nurse and used this cream on the kids before giving them their injections in hospital.
Word of warning though - dont tell the tattooist as she did on her last visit and the tattoo chick got up her and told her she needed to feel the pain.

Well if you want you can change the place where you are gonna get it, there isnt much fat at the small of your back so it will hurt alot mind you that is the reason you see alot of people with tattoos on their shoulders and the back off their shins...it simply hurts less...if all else fails advil, tylenol and all that stuff...NO alcohol!(drinking)

To tell you the truth I just think different people have different tolerance for pain I for one cant stand pain. Before I got my tattoo some friends said it hurts like hell others just said no big deal. But too me it HURT LIKE HELL and I had taken Tylenol but it did not help one bit. It hurt so bad I had to have them stop. Bear in mind it was a small tattoo. When I went to get it finished I drank alcohol. Still the same pain. I had a friend who drank before and said she did not feel a thing. So I guess every person is different.

Nothing. Just be tough about it. I can't see Tylenol helping much. And don't get drunk since they will not tattoo you if they know you're drunk. Plus the alcohol will thin your blood and make you bleed more.
It's not always that bad. Depends on where you get it. Some skin areas are more sensitive.

You don't want to drink, or take pain killers. This will thin your blood and you will bleed more.
Take your shoes off, put your hair up, get a damp cloth for the back of your neck and relax! Tensing up with make it hurt more.... yeah I just had my lower back done three weeks ago. No big deal.
