1. Car stereo (unit and headunit)
2. Car Specific wiring harness
3. Bracket
Thats all you need. Period. If you get the wiring harness for your car you can install it on your own. If you order the Stereo from Crutchfield.com they will send you instructions on how to take your dash apart and how to wire your wiring harness and send you the wiring harness and bracket too. Once installed cruise with some tunes. Some cars might require you to remove internal amps. This is where crutchfields instructions they send you come in handy. Some cars have them in the dash but some have them hidden all over the place (ie: 93 ford explorers are located in the rear panel no where near the door speakers)

There seems to be confusion here between head unit (the actual car stereo) and the fascia (the bit you remove for security). Tell me what car you have (I really wish people on here would give more information) and I can advise you what you need.
After market car stereos are a standard size (DIN E) and have a standard wiring connection (ISO). However not many of the car manufacturers use these standards any more so you usually need a wiring adaptor and often a fascia adaptor.
These are available from companies like Armour automotive and Connects2.

you have to have a whole new stereo. Usually it comes with little tools that you slide into both sides of the stereo to get it out.
Or you could just take it to Best Buy or somewhere like that and they will do it free with your purchase of the stereo

No, you have to buy the whole unit. Each car stereo face fits differently, that's why people take them off to stop thieves.

Go here http://www.metraonline.com for the the adapter harness that connects to the factory harness and bezel kit.
The adapter harness wires up to the new stereo harness, this new harness plugs into the car and the new stereo. That way, when you sell the car, you put the stock back in and keep your stereo.

well you need a whole new unit and adaptor. the adaptor is needed if the head unit plug does not fit with the car plug, its about £10 from halfords. most likely you'll need it, depending if the head unit is new and how old the car is.
