Isotretinoin is a very effective medicine against severe acne. It is a derivate from vitamine A known to be teratogenic, in other words a drug that can't be given to women that are pregnant or not properly following a contraceptive program.
Furthermore, the effects of this medication on the skin are excellent, they virtually efface most of the signs of the severe acne and improves visible the appearance of the skin in a matter of months.
I hope this information would be enough for you, otherwise you can always visit the FDA.gov website.

Isotretinoin is used to treat severe recalcitrant nodular acne (a certain type of severe acne) that has not been helped by other treatments, such as antibiotics. Isotretinoin is in a class of medications called retinoids. It works by slowing the production of certain natural substances that can cause pimples to form.
Please see the web pages for more details on Isotretinoin.

Expat Froggy explains this very well. It's only marketed under the brand name Accutane and must be prescribed by a doctor licensed to prescribe it. (Not all doctors can prescribe, they have to go to a special training offered by the company that manufactures Accutane to get certified.) There is a lot of paperwork to fill out, and it requires monthly visits to a lab to get blood drawn and tested. The usual course is six months, but you can take multiple courses of it, if necessary.
