How would YOU answer this question to a five year old? I am in a loss for words.

Read with him in a simple Children's Version of the BIBLE beginning in Genesis 1:1 through the creation of man. You will not find a reference to it. Then let him know that only GOD knows for certain right now. HE did not tell us. GOD has all of the answers. Your nephew might enjoy you reading more of the BIBLE to him if you don't push it too hard or too long. Other interesting stories for him would be the Flood, the Exodus of Moses, the Ten Commandments, Jesus Walking on the Waters, etc... Have a great time with him. He is only this age OnCe!

Tell him of course they did. You need your belly button to hold your backside on. You'll get a giggle out of him if nothing else.
Seriously: That is a deep question from a 5 year old. Where did he pick up this stuff? Was he listening to an 11th century Catholic debate? Amazing. Better watch this kid: He's got some real brains.

Well...

Adam and Eve were created not born.

So, only babies that are born have belly buttons because they have that umbilical cord deal.

Since Adam and Eve were created they wouldn't have one since they were not born and thus never had an umbilical cord.

I know... I have no factual basis but it's a good guess!

Navels are vestiges of the placental birth of mammals. Since our line has been mammalian for far longer than it has been human, I would have to say that all humans have had navels.
Recent molecular research into mitochondrial DNA have fixed a date for an index female (Eve) as living about 150,000 years ago, and examination of the Y chromosome puts an index male (Adam) at between 60,000 and 90,000 years ago. This sounds like a contradiction, but it is not. An index case like this simply means that the individual was so successful that his or her progeny eventually supplanted all others of that sex. This may have happened over a considerable period of time.

If we are in fact formed in God's image as Adam was supposed to have been then I would expect they did in fact have belly buttons.

Tell him that God took the rib out of Adam through the belly button, and put it in Eve through the belly button.

I'd say no. Belly buttons are for those who were born. Adam and Eve were created.

You know thats a doosey of a question , allowing speculation to kick in I somehow doubt it since these two were created rather than born from a mother .
