No mater what time of year it is, no matter how hot it is, not matter how bundled up my feet are, and even if the rest of me is burning up from the heat, my feet always seem to be freezing. Why?

Poor circulation. I have the same problem, complemented by my legs/feet falling asleep after about 5-10 minutes in the same position.

My acupuncturist suggested cardio exercise and for me to quit smoking.

Give acupuncture a try. You wouldn't believe the total body buzz you get after a 45 minute session. It does not hurt at all.

I suggest you to wash your feet with cold water for few min (approx. 2-3 mins) and keep dry and see the difference. good luck.
