i cant find where to go please help asap

Hello,

ANS) If you have your NHS medical reference number then there are various avanues worth exploring.

No.1 contact your local GP or the family doctor. The one who you or your family is registered with (if you know who that is?). i.e. your local doctors surgery.

No.2 Every GP or hospital is within a health area & has an associated health authority (might not be called a health authority as such any more because of political changes). The regional health authority in your area should be contactable and will be able to help explain how to view your medical records.

No.3 Most hospitals, especially large ones such as teaching hospitals should have a patient liason officer who you can contact and again they should be able to help you see your records. Most hospitals have a dept that deals with such requests, they get them all the time.

You need to ask the physician who holds your file, or if this is in a hospital, go to medical records and make a request. Also, know that there may be a charge for this, that in some cases will be waived if you say that the file is going to another physician. And NO, it is illegal, at least in the US, to withhold a medical record indefinitely; although it may be held legally, pending the fee for copying and sending. Once the fee is paid, if applicable, (likely small amount) there should be no problem.

IF YOU ARE IN THE UK! Go and see your physician, sorry doctor and he will be able to show you your entire file on his screen in front of you. Write down or ask for relevant stuff to be printed as necessary.

You request it from your last physician. An contrary to popular opinion, they can refuse to give them to you, because they own them.
