+ depends on your country. Its just a prefix that indicates that the call is international.
For India it is 00
For USA it is 011
From most of offices in USA it might be 9011.

You didn't say where you are.
Outside of North America you replace + with 00
Within NA you replace + with 011
Doesn't matter if you're initiating the call from a fax machine or regular phone.

The + is replaced by the international access code used in the country you are dialing *from*. If dialling from the UK, and most (if not all) of the EU, this is 00.

The same as you would if you were making a phone call and there was an "+" at the start. You leave it out, but add a double 0 I think. Its that countries dialling code.

You don't type the plus sign. You usually type 00 then then country code. If you were calling UK from overseas you would type 0044 then the number. Get it?
