What the heart would do if the pacemaker was not fitted depends on the reason why the person was fitted with one.

As was pointed out above, the definition of clinical death is lack of a pulse, so your heart pretty much has to stop beating, or at least stop beating effectively. Also, when things go that badly wrong, a host of systemic things happen, blood pH drops, waste products build up, the electrolyte balance in cells becomes imbalanced, so with all that, the heart becomes incapable of beating even if there's a device electrically stimulating it.
