I'd give the dying old woman a ride to the hospital. I don't go for girls so I don't really need the girl of my dreams and my good old friend that saved my life can ride the bus to where ever she's going.

The woman of my dreams (though I am female) because the dying old woman can be saved by the friend that saved MY life. He or she is obviously a doctor.
Then the bus can take them to the hospital.

I would take the bus with the dream girl and give that old friend who saved my life the car to take the old dying woman to the hospital! If it wasn't for your friend, you wouldn't be alive to have the chance to meet your dream girl. After giving your friend the car, the least he could do in appreciation is to drive the old woman to the hospital.
