i`m on my friend`s account and she always says that when she`s on the phone, what does it mean?

"¿Que haces? " means What are you doing? an action but it can means, what do you do? Indirect Object (not appropriate), but better take the first option .

Is not exactly Grammatically correct the tense
because: ¿ Que haces ? it's in Simple Present [The Verb in Simple Form(Haces;"Do")] and if it were in Present Continuous it were like this: ¿Que estas Haci (endo)? it means; What are you do (ing)?

What your friend really ask is just What are you doing in that exactly moment.

Simply, (Spanish)
You can respond her in Spanish too, it could be interesting. "fire against fire", you can ask it here in Q&A what you wanna tell her in Spanish, hehe.

How are you?
If you wanna say "What are you doing" in Spanish you must use "Qué estás haciendo" (comiendo, estudiando, etc.). In the ordinary life we use "Qué haces" for to say "hi, hello...".
Generally "qué hacés" goes together with "cómo estás".
So: "hola ¿que hacés, cómo andas? But this form is used in Argentina, above all. In formal language, we say "hola, cómo estás".

What are you doing? (right now) OR What do you do? (like what do you do, generally, for fun, for a living, etc.)
