tell him you are sorry and you would'nt mind going out with him. If you really do not want to go out with him You can not blame your self for not wanting to go out=( It took courage for him to ask you and he probably feels terrible so if you would'nt mind going out with him you should reconsider.
It is ur choice though remember a pity date is worse then no date only go out if you kinda like him. otherwise act like nothing hapened when you see him agin he will be gratefull=)

either wait for him to ask again, which may or may not happen (would you really want to date someone who asked you out 3 times?) or ask him out. I think he's appreciate your initiative and interest. Good luck.
