Have you considered getting with him either before hand or after wards for something special. Maybe you have him take you to dinner or something. Just because he cant be there doesn't mean that he does not want to. Sometimes things come up out of the blue or people have other obligations that they can not get out of.
If you really want to do something with him for your birthday talk to him. Maybe he already has something in mind or something big is going on with him and he complete lost track of time. Communication in this situation is the key.
I know from personal experience my birthday every 4 years falls on a holiday where I spend my time mainly with family. Its not that I don't want to spend time with my girlfriend it's just that my day is so hectic that we normally just get together on the day after.
