It means you got busted doing something you were not supposed to be doing. Like getting caught cheating on a spouse or significant other, getting caught looking at porn, or getting a cookie out of the cookie jar when you were told not to have any before supper.

well there are usually goodies in a cookie jar so if one says to you you got caught with your hand in the cookie jar they are saying you went after something very juicy and not your business and got caught!
