I would want to know which specific club you're referring to.
That would tell me a lot.
One way that you could thin a shot and take a divot is with a club that has a wide bounce on the bottom, like a sand wedge.
What happens in this case is that you hit so far behind the ball that you're catching the ball thin coming out the upswing of your divot.
And usually is it a shorter club that you took too far inside on the back swing that you can not get it square to the ball in time, thus you hit well behind the ball.

you cant really if you hit it thin you generally pick the ball clean and hit the top portion of the ball creating a shot with top spin.
when you take a divot its either before the ball a chunk or after the ball solid contact.

it is not really a divit it is more a streak mark made by the ball. some of the times when you hit it thin it is going closer instead of away from the ground. so it just tips the surface of the grass in front of you and the velocity from the ball moving takes some of the grass away from the ground along with some mud as well.

by the sounds of it you may be transfering your weight too soon on the downswing,also if when your at the top of your swing if your looking over the top of your shoulder your gonna come in steep and hit way behind the ball so try to have your eyes over the middle to bottom area of your deltoid and this may help you get a better contact.this is the best advice i can give without seeing your swing,hope it helps!

This is because the club head is still on a descending arc at impact but the "release" of the wrists has been delayed for too long (the hit was too late!!)

It could be player is digging down to hard in the through stroke and not sweeping the ball away as it should be or maybe the ball positioning needs changing in the stance.
