Royal Caribbean and Celebrity sail from Cape Liberty (NJ).
Carnival, and Holland America, sail from Manhattan.
Princess and Cunard sail from Brooklyn.

Pretty much all of the other North American majors. Royal Caribbean and Celebrity out of Bayonne NJ (port liberty or something to that effect...), Holland America Line, Norwegian and Carnival out of Manhattan, and Cunard (QM2, not sure about QE2 though) and Princess out of Brooklyn .
