That depends on the person.
In Chinese medicine terms, a good thing to do is drink some warm water and do some light stretching and exercising to get your body systems moving. (Cold water is said to chill the organs & cause ill effect)
I know a Yogi master who lives near the ocean & every day he goes for a dip in it first thing in the morning. EVERY day. Even in the middle of the winter (in NY)! Very invigorating. I think it's really built up his physical stamina.
If you are looking for routines to help you manage your life, check out flylady.net. She has an entire system for organizing your entire LIFE broken down into 15 minute increments. This is the system that I follow. I developed my morning routine through using her system; tweaking it to suit my life.
I think anything that gets you going and feeling alive first thing in the morning is great for you. (Excluding drinking coffee or other substances that tax the body of course) Stretching, drinking water, going for a walk, eating healthy nutritios food, meditating...
Generally, in my experience, it's best to get out of bed as soon as I wake & get my day started. If I just lay there, that feeling of laziness and foggy tiredness carries throughout my day.

Immediately after I wake up, I hit the snooze alarm, and go back to sleep. I usually repeat this process about 12-15 times on average.

If I am the one being asked first thing to do is to praise GOD thanking him that you are still alive that you have additional one day again in your life. Start the first minute by prayer and after giving thanks and praising GOD and your supplications proceed to the normal things. And that will add complete your life.

Not to get reported, but immediately upon waking, I masturbate, to get stuff flowing again.
Then I exercise.
Once college starts again, I'm going to add meditation to that list, just so I don't wind up killing anyone.
And to add, just so it's known, I shower after all of that.

Stay in bed, allow yourself to awaken and absorb into the day smoothly, and reflect a few moments before getting up. ~ ~ ~ Be in the moment before starting your mind to thinking of your day's responsibilities. . . . If in a relationship...move your hand to feel your sweetie and wish him / her good morning.
O.k. . . . now can get up and start your day.

get out of bed and do one thousand push ups and sit ups and then go outside and run for about ten miles and then lift weights
if it doesn't kill you then you will probably starve to death because you wont have any time to go to work and make any money
but you will be a lot healthier

First thing you should do is thank God for another day and for all the things he has blessed you with.
