I have always pulled my foreskin back, placed the condom, then rolled my foreskin back up over the glans to allow normal movement. I have heard of it both ways, though, so to each his own, I guess.

It's perfectly normal. Sometimes pulling the foreskin back can be uncomfortable - as it's highly sensitive.

I've seen that before. My ex said that it hurt him to have the foreskin pulled back. You could always ask him why he does it.
