i would use the sandwich to lure a very large bird...when it swooped down to get some of the sandwich, i would grab its legs and let it fly me away, off the island, to a place where they had milk AND peanut butter...

Id probably journey to the abandoned village to find some shelter and sit in the shade until sundown. Because I'm alluding to your "deserted" description, I'm assuming that there are no animals. Id go and look for coconuts and hollow it out, drink the milk while eating the peanut butter sandwich and search for ginger and other nutritious roots for a harvest.
