The fact that they are widely used in predominately Muslim countries make me think that it is not prohibited.

It depends on what you're smoking with it. If you are smoking tobacco through the device, then its definitely wrong. Allah swt would not allow something that damages your health and makes you a less productive and helpful member of society. I don't think you should look for some ultimate answer to whether its allowed or not, just use your common sense: do its benefits outweigh its risks? Could you find a better pasttime? Are you going to waste valuable time with disreputable friends going to a sheesha place? Are you going to prioritze it above prayer and family?

Not if it's 'holy smoke!' The trouble with all holy tracts is that they could not possibly take into account later innovation. As far as I know the Q'uran forbids only drinking of fermented juice of the grape, but not vodka or whisky(probably unknown at the time in Arab countries). So the customs and practices of any religion are open to interpretation and dispute.

I think its haram as:
In the Qur'an, Allah says, “...he [the Prophet] commands them what is just, and forbids them what is evil; he allows them as lawful what is good, and prohibits them from what is bad..." (Surah al-Ar’af 7:157). So what are the evils of tobacco use, for your health and for your deen (religion)?
1. Danger to your health
Allah says, "...make not your own hands contribute to your destruction..." (Surah al-Baqarah 2:195); "...nor kill yourselves..." (Surah al-Nisaa 4:29). It is universally understood that cigarette smoking causes a number of health problems that often ultimately result in death.
2. Waste of Money
Allah says, "...But spend not wastefully (your wealth) in the manner of a spendthrift. Verily spendthrifts are brothers of the devils...” {Surah al-Israa’ 17:26-27} And in an authentic hadith the Prophet (peace be upon him) said that: "Allah hates for you three things: gossiping, begging, and wasting money."
There are many more resons aint listed them all would take ages but to get to the bottom of it i think all of the above you have listed are not good and personally for me are haram, but Allah knows best.

depends on the views of the Muslims who answer this question. I am not muslim but i have had 2 muslim couples and both of them smoked hookah, even if one of them seemed to agree that it is haram.
however, as the first respondant said, many muslims in arab countries smoke it so... it is a light sin.

Tobacco is an intoxicant, so technically it is forbidden like alcohol and drugs. But it doesn't have the same affect on behaviour like other drugs have, I think this is why some people today say it's permissible.
