Ok, I actually researched this for you.
NO you do NOT need a passport from the USA to Puerto Rico. Since it is a US Territory, it is not required to have a passport to enter and leave P.R. from the USA.

Not if you are an American citizen. Puerto Rico is a territory of the US, so you will just need a state ID or driver's license. The rules are the same as traveling to another state.

You don't need a passport to go to Puerto Rico as it is a US territory and as long as you are traveling form a US state you are not considered to have left the US.

I am a American citizen and I never did
Puerto Rico is a United States Common Wealth Country
Just like Guam, Samoa, and The Virgin Islands.
So I really don't think that you do need a passport even after 9/11.

Starting on January 1st, 2007, yes you do.
I was surprised to learn this as well - the only places you do not need a passport for are Mexico, Canada and the Virgin Islands.
