Some possible reasons:
Cheap dye. This isn't universal-- it depends on your hair and on the color you used. (Especially black dye-- even the cheap stuff tends to hold on.)
Red dye. It fades much faster than, say, brown.
Your hair wasn't porous enough. Some people have a hard time dyeing their hair; it simply won't "take" very well.
Your hair was over-processed before you dyed. I had hair that was bleached too many times, and simply wouldn't hold dye.
Chlorine or other junk in your water. When they chlorinate the water here, it strips any color out of my hair about twice as fast.
Using too many products, especially those with alcohol in them.
Not conditioning your hair. I find that when I don't condition, I lose color faster. Conditioning helps seal the hair cuticle shut, which also makes it shiny and smooth.
Hair-color-fairies have been sucking it out of your hair while you sleep. Alright, I made that one up. Seems like that's what happens sometimes! Better luck next time.

The reason is that you probably used a rinse which will come out every time you wash it. I would add some indian henna to it next time to deepen the color and make it last a little while longer.

Because cuticle is damaged and porous, probably due to the coloring. You need to repair and re-hydrate your hair before you color it again.
When the hair is damaged, its open and split and threrefore cannot lock in the color. When you do try to color it again, go for low ammonia and low peroxide, preferably a semi-permanent.

Was it a permanet color? Did it mix with peroxide? or was it a wash in color? Also check base of shampoo-- if green base, will reduce red colors; violet base works good

maybe that shampoo doesnt work with ur hair , it would be advisable to discontinue theuse of thisbefore things get more severe like hair breakge

don't worry about the die, though i would go to a hair slaoon to get it died with the good stuff. some hair wont keep a die, i had to die mine every moth or less to keep the color. filaly i gave up and let it grow out.

that usually happends because i could say the product was not good cuality of your hair did not take the product good, dont worry i bet it looks great!
