Pectorals, triceps, lats, some shoulder as well. Pushups and crunches can help to build your upper body as well. If you don't have any workout equipment also try free squats, or squats w/o weights. No matter what type of shape your in, a few sets of those to exhaustion will help build your legs up. Also, pull ups will help out.

200 reps is overkill, 3 sets of 15 to 30 reps is good. the pushup doesn't develop the whole upper body, although a good proper push up uses muscles in the entire upperbody, it builds mostly the tricepts

Basically, Chest and triceps and yes, especially with progressively increasing the resistance, rest, proper stretching, good form and a high calorie healthy diet, but a two count each way with breathing will do it two. Remember to breathe!

Pectorals, triceps, and deltoid a little. The closer your hands are together, the more you work the triceps.

primarily the pectorals (chest)
secondary the anterior deltoids (front shoulder) and triceps
yes...
also, concentrate on good form, you will see the best results this way.
