medically speaking....
My boyfriend seems to think he looks slim when he looks in the mirror when he's very much NOT....similiar to all those big girls that wear short skirts, short tight tops and still think they look great.

I think it could be a couple things,

1- Denial, they know it deep in side, but you would never catch them saying it.

2- Overcompensation- They know it, but they want to do anything and everything in their power to change it, irrationally.

3- Ignorance. just plain ignorance

4- Fear, Sadness, or Shame

Maybe he just has a distorted body image. What makes you think that it's a medical condition? He may just be an over weight, and if you are not happy with how he looks may you should think about the relationship.

Bigorexic. Weightlifters no matter how huge they get think they are pathic weak and little. Very common with bodybuilders. It's actually a real condition and you can read about it here. http://menshealth.about.com/cs/menonly/a/bigorexia.htm
