Do i have to go back to my country to get my extension beyond 3 years for my H1B visa?If yes ,then when do i do it.

If you have an H-1B, your employer must file for an extension. It can take months so I recommend you do it with plenty of time before your current I-797 expires. If not, you'll have to pay the $1000 Premium Processing fee (it'll guarantee a decision in two weeks).

If your employer doesn't want to extend you, you have the option to change employers. Again, this must be done before you end your job or your H-1B expires.

You won't be able to leave the country without a valid I-797. If your visa stamp (in your passport) has expired, you'll need to go to the embassy when you leave the country with all of your documentation proving that you're legally employed.

You can be on an H-1B for six years. Under some circumstances that can be extended (ie. you're in the middle of applying for permanent residency). For any exceptions to the normal rules, I'd recommend that you hire an attorney.

A visa is for entry, not for length of stay. So you extend your stay in the US by filing form I-539. If you leave the US, you must apply for a new visa in order to re-enter. To do that, you must have a copy of the valid, current I-797 showing a new date.

You have to speak to your employer and get it extended. Or have them sponsor you for a Green card. Since H1 visas are employment based
