1. Undergo some spoken English course
2. Read English newspaper
3. Read English books.
4. Talk to your friends and family members in Inglis even if it is not correct always. Try to express every thing in English.
5. Try to think in English while speaking. Not that thinking in your mother tongue and then translating in English.

Get a copy of Wren and Martin 's English Grammar with the Companion Volume for exercises.After improving the spoken language a bit, enrol in British Institute's Course in English as a Second Language.

To improve your vocabulary i think you should read a good books be it story books or any other good book. While reading you will definitely come across certain word's ,whose meaning you would not be knowing ,then in that case pick up a good dictionary and search for the meaning of that word. One more way to improve your vocabulary is simply reading through dictionary everyday ,sincerely.Then, always try to converse in English ,never hesitate if you are speaking incorrect English because you learn when you make mistakes.
One more thing is that you should always think in English while you are speaking in English, never think in Hindi and convert that into English.
I am sure if you try hard you will be able to improve your english.

The Tutorial can be altered to Your Mother Language , There are a Selection of 33 World Languages Available.

if u like reading try to read lot first if u don't know the meaning of the words try 2 look it in the dictionary 4 couple of days until u fill much more confident with the vocabulary u have then try not to use it read and skip the unstable word and u will understand it by urself by understanding the story or whatever u read

Your question was written better than many people who have attended 12 years of public education, so I have a feeling you are better than you realize.
However, you may want to check out a book from the local library every few days. Don't be afraid to check out 'readers' ... books designed for elementary aged kids to improve their reading skills. Read them & then try to incorporate some of what you have learned into your conversations.
I don't know what you native language is, but you may want to cross-learn with a student of your native language. When I took Spanish, I worked with a wonderful woman named Maria. She was learning English, as I learned Spanish. I met her at my college, where she volunteered to help students trying to learn Spanish, in exchange for assistance on learning English. It was a win/win situation for both of us.
Of course, as already mentioned there are ESOL (English as Second or Other Language)classes available.

sign up for ENGLISH AS A SECOND LANGUAGE (ESL)courses. they do wonders, i work with a lot of African immigrants to the US and thats where I have then go first to assimillate in the US society, it works wonders.. Also read lots of books and watch foreign english movies some are subtitled, which would help in translation, but hearing on how words are being used works as well.

Read newspapers and novels or even English comics. Try to use the words and phrases in your own speech and text. Talk to yourself in english. Forget that you are weak in it.

Only one thing, short of one on one contact with English speaking persons, is to READ, READ, READ.
Read everything you can lay your hands on, magazines, papers, but especially good books. If you are a beginner, good children's books are ideal, because the language is simple, yet the grammar is correct.
