I happen to work in an icu where kidney transplant patients go after a transplant. We also care for many patients who are brain dead on arrival or become brain dead after a traumatic injury or from a bleed in the brain. There are different kinds of donors. Some are what we call living related donors who are people that choose to donate one of their healthy kidney's to a relative who is in need of a transplant. In these cases the recipient (of course) always knows who the donor is. The patient and donor still have to go through with the blood and tissue typing that any other donor/recipient duo have to go through.
The next kind is called the "designated organ donor". In this case a patient is declared brain dead and pronounced dead by a qualified physician after a series of tests. The family at that time have more than likely been approached regarding the choice to donate their loved ones organs to another individual who is in need. A lot of the general public are not aware of this but the family actually has the right to donate organ or organs to a specific individual who they know that is need. The organ donation team will then contact the recipient and proceed to do the appropriate blood and tissue typing necessary in order to make sure that theirs is a match. I do not know all of the details about tissue typing for organ donation but do know that there have to me less specific matches for kidney donation than there is for other organs such as heart/lung, liver and pancreas transplants. Amazingly enough most of these designated donor cases are a match for kidney transplantation. In these cases the recipient is usually aware of who the donor is but if the family of the deceased does not want the recipient to know then it is kept confidential.
The next kind of donor/recipient is the patient who is pronounced dead and the family chooses to donate organs. If the family does not say they want an organ or organs to go to someone specific they know who has a need and is a match then this is kept confidential.
As a nurse it is a very rewarding experience to care for a patient who is pronounced dead one day and the next day get to care for the very patient who is the recipient of an organ gifted to them by the family of the deceased. One day I am grieving with the family (and after 21 years of nursing I do still grieve the loss of life even if I have just met the patient and family) and the next day get to actually see a living part of that patient helping someone else to have a better quality of life and get to rejoice with that patient for such a blessing they have been given.
In my opinion organ donation should not be "open" in that of what we call cadaver donors. If the deceased were a family member of mine it would give me great comfort in knowing that part of them could go on living through another human being but if I knew that person and they were not compliant with their medications or did not take care of the gift that was given to them I would have a very hard time seeing part of my loved one die again because someone who was given the gift of an organ took advantage of it. I would however like to know general information such as their heart went to a 25 year old with severe heart disease who now has a chance at a better quality of life or something general like that.

I couldn't just sit around and do nothing like my doctors suggested.
They didn't want me to do anything or to take herbs or herbal remedies, but I had to try something - they just wanted me to do dialysis!
This program allowed me to take control of my health. I went from Stage 4 to Stage 3 kidney disease.
