The response from Sandy was correct. If you have't spoken for a while, starting up a conversation may be somewhat awkward. So, if it has been a while, just start with "hello".
Tell this person that you realize that it has been a long time since you spoke and you miss them. Meet for lunch, or any period of time that can be set aside for a conversation to get you caught up with each other.

Reach out via email, text or written communication first since it can assist with how you want to start the conversation. (I know its been a long time..sorry I havent been in touch...I miss you...) and wait. If they are open to you they will reply, then take it from there.

im in the same position as you. its with this girl i really like and we talk online and whatnot but im about to call her up. its going to be akward because we havent hung out in a while and im not sure how she feels about me but im just going to see what shes been up to, how her christmas was, etc. just make small talk and go with it.

you just do it. There is no way to get rid of the awkwardness. If it will be too awkward, then don't call.
