I really think it is dependant on your state and you carrier on how much is charged. I'm in NM with Qwest and I am charged $.75 each time I use *69. My carrier also has options when using it that will ask you if you want the number to be dialed.

at&t charges $1.99 without Call Return(*69),and $5 a month for the service,and no I think you can only call the last person.
