Revelations 21 says that a new Heaven and Earth will be created and God and Jesus will rule from the new Jerusalem, but your question seems to be, will I go to Hell or not?
The people that don't go to Hell are the ones who's names are written in "Jesus' (the Lamb) book of life, (Rev 21:27). This book was also mentioned in the old testament. Moses asked God to erase his name from it if He didn't forgive Israel's sin, (Exodus 32:32).
So, what will make God take your name out of His book?
In Exodus 32:33, God says sin, or missing the mark, will. So, how do you not sin? Psalm 119:11 says to hide the Word of God in your heart, or to know it so well that it becomes your way of living.
Rev 22:19 also says that if you take away, or ignore, any part of the things talked about in the prophecy, your name will be taken out of the Book.
Rev 3:14-18 says that Jesus will reject people that are only lukewarm towards Him. These are people that are deceived, thinking that they are "OK" with what they have, and don't need to study the Word, or pray. They are just as bad as the people who reject God totally.
In Matthew 10:32-39, Jesus shows how serious He is about this. He says that if you put your relatives before Him, you're not worthy of Him. Meaning, if you take other people's opinions rather than listening to what The Bible says, you aren't a true follower.
So, who will be in the book?
Before He came to Earth, Jesus was called The Word, (John 1:1-5). And in verse 12, It says that people who receive Jesus (the Word), or believe in Him, were given the authority to become children of God.
Matthew 7:15-20 tells you how to judge between a true prophet and a false one... by their fruit. The same can be said about a true Christian. They will show fruit, or evidence of their Christianity. That's why in verse 21 Jesus said not everyone that claims to follow Him will enter the kingdom of Heaven... only the ones who obey God. He goes on, in verse 24, to say that whoever hears and obeys the Word of God is wise, and the ones that don't obey are foolish.
Rev 3:1-5, the church in Sardis isn't living like they should be, and Jesus says that the ones who continue to live right, and overcome (sin) won't be blotted out of His book.
So, hopefully, this will give you an idea of who will "go to Heaven", and who won't. Please continue studying it out. There are a lot more scriptures that address this issue. For instance, there are more books, in addition to the Book of Life, that men will be judged out of. If you don't have a Bible and concordance, you can download one from www.e-sword.net.

Belief in God and Christ should be acompanied by action, and believing is more than Acknowledging. If you no you beleive in Christ sometimes that doesn't cut it because in order to go to heaven you must abide by the Bible, and do his word.
"But if you refuse to listen to the Lord your God and do not obey all the commands and laws I am giving you today, all these curses will come and overwhelm you"(DEUTERONOMY 28:15)
Baptism signifes repentance, and all followers of Christ should be baptized, and Baptism is closely linked with the renewal of life and a changed life.
And for church its the cornerstone of the church, Christ is the head of the church and Satan works against the church.
JUst keep in touch with the Lord more...and get Baptised and become a Born-Again Christian....:)

The Gospel of Thomas – The one the Church rejects… , "Show me the stone that the builders rejected: that is the keystone."
“Split a piece of wood; I am there. Lift up the stone, and you will find me there”. This means God is everywhere.
"God does not live in houses of wood and stone but in the hearts of men ... " This means you don’t have to go to church to pray
The Church is a billion dollar business, God can hear you with out the aid of a priest.
…But without you donations where would the Pope live.

yes, I'm sure you'll go to heaven (barring you haven't lived your life disrespecting everything God created). You've accepted christ and some say that is all it takes. Some believe differently, but we live in a world where there are hundreds of religions all with different ideas of the truth. I believe when you die you will go where your soul believes you should be.

Church has nothing to so with it. How is your relationship with jesus? For some people, Church helps their relationship with christ, and and for some, it makes it harder. It depends on who the person is. Just make sure you have a good relationship with jesus, and you should be fine. No, you will be fine. Just because church isnt important, doesnt mean that christ isnt. Jesus should be the center of your life.

Ps. 37:11: “The meek ones themselves will possess the earth, and they will indeed find their exquisite delight in the abundance of peace.”
Rev. 21:1-4: “I saw a new heaven and a new earth . . . I heard a loud voice from the throne say: ‘Look! The tent of God is with mankind, and he will reside with them, and they will be his peoples. And God himself will be with them. And he will wipe out every tear from their eyes, and death will be no more, neither will mourning nor outcry nor pain be anymore. The former things have passed away.’”
Mic. 4:3, 4: “They will not lift up sword, nation against nation, neither will they learn war anymore. And they will actually sit, each one under his vine and under his fig tree, and there will be no one making them tremble; for the very mouth of Jehovah of armies has spoken it.”

There's no easy way in or out my friend. You really need to find The One Eternal God. Why do you think He put us all in this world in the first place?
I pray that God simplify your affairs for you.
God knows best.
Peace and Love.
