I think they can, its all a matter of perception, maybe they hear him but what hes saying and what hes thinking are two different things, maybe when he says " Death to you vile women" he's actually thinking it and saying something else, baby talk maybe, this could be how we all grew up and we don't even remember it, and its Lois not Louis

The only episode where they totally understand him is the 2 part episode where stewie kills lois and then ends up getting killed when he takes over the world. They do understand him, because we find out at the end that the whole thing was just a simulation. The only person who understands Stewie is Brian. Other times, the characters will seem to respond to Stewie, but its really just like anyone would respond to any baby when they are babbling.

Peter and Lois think (or really want to think) that Stewie is just an ordinary baby who doesn't want to slaughter them. I think they believe so firmly that he's normal that they refuse to accept his mean words. May I add that in some episodes, Stewie says something borderline to Lois and she understands him perfectly fine but just doesn't take him seriously. See what I mean by watching the first few episodes of family guy.

Stewie speaks "baby talk." Only dogs understand that.

Some time they seem to understand other times they dont.

Personaly I think they hear him but zone it out like most parents do.

I used to see it all the time the kid is talking and the parents (for good or evil) dont seem to knowtice because the child is aways talking and never really say anything. It just becomes cute baby gibberish to them.

maybe because stewie's level of understanding is superior than theirs.... so they can't understand what the heck he's saying
