A lot of fish just get old and slowly die. The only thing is, if it is still alive and you leave it in your tank till it dies, it may be harmful if there are other fish in the tank. A lot of fish release toxins when they die contaminating your tank. So if it remains in the same condition for a little while I would suggest removing from your tank before it dies.

I don't know too much about diseases. I have been keeping fish forever but I don't get a whole lot of disease in my tanks. It sounds to me like you have a water quality problem. Test your water and make sure you don't have high ammonia, nitrite, nitrates. And whether you do have a problem or not, do a 50% water change and see if that helps him. I dont know if you can do anything at this point to help your fish, in my past experiences fish that are laying on their sides generally dont last very long I have had that happen to angel fish and sharks and it's never a good thing-sorry. . . .

check if its gasping for breath if it is get a bigger pump or clean the diffuser block on the 1 you,ve got in it. when there's enough oxygen in the tank it should make a remarkable recovery if i dosnt start swimming after about 20 mins you,d best check for a pulse remember 5 heart comps to 1 breath

fish normally do that, or with other things, or just sleep on its stomach on the ground. its normal sleeping habits. but if it goes to the surface and doesnt respond it is dead. coz 5 years it quite old.

when my fish did it we cleaned the tank super good and let the water sitt out for like 30 minutes and then he was fine. our fish is about the same age.

its proboly sleeping..
or if you see it trying to get out its fin is stuck under a rock
or.. its maybe scared of you.
