I had one of these done.
It was completely painless, and they use a gel that covers the entire scrotum. I was in there for about 30 minutes. It was done by a woman, and she was very discreet about it. The ultrasound tool is moved over your scrotum from side to side, and no pressure is applied at all. It is not uncomfortable in any way.
She had me lay my penis on my stomach, and she put a towel over my penis and stomach, and surrounded the scrotum with cloths so that only the scrotum was exposed. Again completely painless.
I was fine until about 25 minutes in, at that moment I got an erection (the procedure isn't sexual of course in any way, sometimes it just happens, you get an erection). So I layed my arm across my penis under the towel, to keep it from popping up. She was, again, very discreet about it.

I've had a testicular ultrasound, before. This is the way it went down.....I was handed a paper gown and instructed to remove all my clothes, put on the gown and lie down on an examination table. After a few minutes, the ultrasound tech entered the room. The tech was female. I had been worried about that! She was very professional. I was laying on my back. She dimmed the lights in order to see the ultrasound screen better. She squirted some ultrasound gel onto my scrotum, after raising my gown to my waist. The gel was slightly warm. Next, she placed the ultrasound doppler on my scrotum and began moving it around to get a good pic on the screen. That lasted for a few minutes on each testicle. It's painless. Although, I doubt that was your biggest worry! I didn't get an erection. I didn't feel any pain. I'm sure your exam will be the same. Don't worry, man.

What are you worried about? The ultrasound or the results?
The room will be cold. Your penis will get as small as it can and your scrotum will tighten up. The ultrasound tech will have you raise your hospital gown up for access. They will ask you to hold your penis out of the way or they will tape it out of the way. The tech will put some goop on the sensor and start rubbing it (non sexually) around your scrotum. You won't get an erection and if you do, it happens, it will be covered by your gown. No worries mate, we've seen it all! You'll clean yourself up and go home. Don't ask the tech for the results; the MD has to give you the news. You'll hear from the MD after a radiologist or urologist interprets the findings. You'll receive a clean bill of health or something unimportant or you'll be in for a removal of a testicle.
Ultimately you will be fine.

I think it would be best for only those WITH TESTICLES and have had the ultrasound to reply, since there is no way in the world that anyone else can give an actual answer otherwise

I'm a nurse. Breathe and try hard to relax, everything is going to be OK. It's painless if you didn't know. The tech tells you everything he/or she is doing. Let the tech know you are nervous and they will relax you. You will do just fine. See yourself three weeks after the ultrasound, that usually helps me when I nervous about something. Picture yourself already done with the procedure. Take care.

I had one done by a female technician, it was a little embarrassing. It took a good 30 mins. she looked at her computer screen more than she looked at my testicles. After about 15 mins. she rubbed the wand up and down my penis, I asked why she was doing that and she said she was trying to get blood into the area for a better picture. It felt good
