Local Post Office or County Clerk has applications, you need a birth certificate and a valid drivers license unless you are under age.

It depends on where you live. You can go to this site and download the forms you need to fill out for free: http://travel.state.gov/passport/passport_1738.html Make sure you use that government website. There are other sites out there that will allow you to download the forms and give you the same information, except they want to charge you for it. The government site is free. I live in a very small community so I just took the completed forms, my drivers license, birth certificate, and passport photos to our local court house. The magistrate saw that everything was in order, collected the fees, and mailed it off. It takes 6-8 for them to send you a passport after all this, unless you paid extra for the expedited 10-14 day service.

First go to FedEx Kinko's for your passport photos (photos are in color). Next, go to http://www.travel.state.gov/ and download and fill out an application, but don't sign it yet. You'll need proof of U.S. citizenship (birth certificate), and exact fee of $97.00. Take it to the passport agency found in http://www.travel.state.gov/ , it will give you all passport agencies in your area, just click on "where to apply for a passport" . Some agency will accept application by appointment only, call first before you go.

The passport application fee is $67. The execution fee is $30. The total is $97 .

I went to the post office for mine. When you go-take a copy of your birth certificate and a passport sized picture. It will take about 3-8 weeks to get it. So, plan ahead.

I went to a post office.
But not all of them do passport pictures.
Go to your nearest and ask them if they do passports and if not, they will probably redirect you to one that does.
There, you can just ask the worker at the counter, and he/she will help you with the rest.

I got mine at my county Liscense Center, I also saw you could get one at some post offices.
